Cardiac Rehabilitation
A comprehensive wellness program promoting heart recovery using a positive approach to help you achieve your lifestyle goals and maintain heart healthy habits for life. Our program enables YOU to take immediate action towards improving the course of your life through clinically supervised exercise and risk reduction program. This program is designed for individuals who have had:
- Heart attack
- Heart surgery
- Angioplasty
- Stent
- Stable angina
- Congestive heart failure
- Valve sugery
- Other heart problems
Cardiac Rehab involves the development of an individualized wellness plan for you including short and long term goals, a commitment to attend the risk factor modification classes focusing on overall wellness through nutrition, stress management, exercise, and prevention and a commitment to exercise 3 days a week in our program. Our telemetry unit monitors your heart rhythm as you exercise. The program offers you an opportunity to meet with other heart patients and share experiences including meaningful time to address fears and concerns with qualified professionals. The Cardiac Rehab team includes registered nurses, exercise physiologists, respiratory therapists, registered dietitians, your physician, physical therapists, and social services as needed. Medicare and most health insurance policies cover the program. It is important to check with your insurance company before you start Cardiac Rehabilitation in order to confirm coverage.

LEAP into Health - A Lifetime Effort Aimed at Prevention
Formally Healthy Hearts and the STAR program, the LEAP program serves as a transitional bridge between medically supervised clinical therapy & independence. The program is for individuals who would benefit from exercise, nutrition education, stress management; and how to fit healthy habits into any busy schedule. Whether you have medical problems, have recently completed medical treatment, or you feel like you’re teetering on the edge of a chronic disease, this program is for you. We work closely withyou’re your referring Physician & Physical Therapist to determine limitations & special considerations related to your client’s health. The fees of our program are dependent upon the length of membership prescribed and the amount of individual attention required. Anyone can participate, with or without a chronic disease, however, a doctors referral is necessary. More Info

Heart Failure – Management & Exercise Program
This year long program provides an atmosphere conducive to improved functional capacity, risk factor reduction, and symptom management support through supervised exercise and educational sessions. Participant must have a diagnosis of heart failure and a physician referral for the program.

Pulmonary Maintenance
This class is structured for individuals with COPD, or other respiratory conditions who would benefit from assistance with exercise instruction from a licensed respiratory therapist. Participant must have a physician referral for the program.
